ATHLETIC PHYSICAL EXAMINATIONS:

All athletes planning to compete on any team at Newbury Park High School MUST have completed a physical examination and have on file BEFORE you tryout or compete! You can download a physical packet from this site, click “Athletic Packet” (below) or pick the packet up from Mrs. Spaulding in the main office on campus.  Please note that there are three forms that will download.  Please turn in ALL forms into the Athletic Office only! BUS FEES must be paid BEFORE the first contest.
